Birdie Break has been contracted to provide childcare services to the ICSE 2025 attendees. The organizers of ICSE 2025 have passed your email to us so that we can gather more information and prepare to care for your child or children.

I/We, the undersigned, are the parent(s) of the below named child/children and we agree, in taking advantage of this child care services, to release and hold harmless Birdie Break from any and all claims, demands, suits, cost, and charges in connection with or arising out of the child care service, including, but not limited to, bodily harm or injury to our children.

We agree to release and hold harmless Birdie Break from any and all claims for damage to or loss of any of your property resulting from or arising from being present on the premises.

I hereby grant permission for Birdie Break and its Birdie Sitters full authority to take whatever actions they deem necessary regarding my child’s health and safety in the event I cannot be reached or in the situation where time is of the essence; and fully release Birdie Break and its Birdie Sitters from any liability in connection with those decisions, I grant permission for emergency treatment by a rescue squad, private physician and/or hospital or emergency health care facility staff if needed. Any such action will be taken in the best interest of my child and will be reported to me as soon as possible.

*Meals will be provided but please bring snacks if your little one is particular about food.

*Location for childcare is room 102 at the Rogers Centre: 55 Colonel By Drive, Ottawa, ON K1N, Ottawa, ON K1N 9J2, Canada

*Phone number: please include the country code, and ensure you will be reachable at this number by text and voice during the conference so you can be reached if concerns arise regarding your child.

*We will only be accepting children from Parents who have registered for ICSE 2025 and who have indicated with ICSE 2025 that they are seeking childcare. Please therefore use the same name and email address that you used to register for ICSE 2025. We reserve the right to communicate with the organizers of ICSE 2025 if your information does not match the information of a registered attendee.

*We will report to ICSE the number of people who have completed this form, the number of children under care, and the time periods where they require childcare, but we will not release the names of your children or other details about your children to the conference organizers. Please see our Privacy Policy.

*Following registration, we will re-confirm your registration closer to the date of the conference in mid-April.
